• Act as a key liaison between clients, their families, and outpatient providers, ensuring comprehensive care is provided throughout their treatment journey.
• Coordinate with insurance companies for pre-certification and utilization management, advocating for client needs in a complex healthcare landscape.
• Engage in meticulous discharge planning, collaborating with clients and multi-disciplinary teams to ensure they have access to the necessary resources post-treatment.
• Provide therapeutic meal support, wherein you will model and encourage healthy relationships with food, helping clients navigate their dietary challenges.
• Deliver empathic and affirming care tailored to the specific lifestyles and identities of each client, addressing the complexities of their individual circumstances.

In this role, you will primarily work with a diverse patient population, including older adults facing a variety of mental health challenges. Your expertise will help address nuanced issues such as depression, anxiety, and adjustment disorders, allowing these individuals to find effective coping strategies and promote emotional resilience. Your cultural competency will be essential in understanding and responding to the intersecting identities of clients.

• Master's degree in Social Work from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of New York is a must.
• Proven experience working with older adults, with knowledge of age-specific mental health concerns and interventions.
• Strong clinical and diagnostic skills, with the ability to develop comprehensive treatment plans tailored to individual client needs.
