Job description

Warrant Reduction and Advocacy Program (WRAP) Case Managers are responsible for answering and facilitating compliance referrals from San Joaquin County Probation Department and providing relevant case management services to those referred.

MINIMUM QUALIFICATIONS:
Two-year college degree from a community college in Criminal Justice or Social Sciences
OR two years' experience with case management and/or group facilitation in the areas of criminal justice or social service
Certified Community Health Worker or the ability to obtain certification within 12 months of hire.

DESIRED QUALIFICATIONS:
Bachelor's degree in a related field
Bilingual English/Spanish
Work experience within a correctional facility, community corrections, or with a similar population

RESPONSIBILITIES:
Process compliance referrals from probation.
Initiate communication and services with referred clients.
Assess and assist with addressing client challenges and barriers to maintaining compliance with probation requirements.
Assess client strengths and needs in areas such as employment, medical/mental health needs, housing, transportation, legal services, education, etc.
Develop case management plans and ensure ongoing monitoring and evaluation of progress and effectiveness of case plans.
Conduct home visits, meet clients off-site when necessary, and assist clients with resource navigation.
Facilitate reentry and family support groups.
Maintain an open line of communication with Probation Officers and provide Probation Officers with monthly updates regarding clients.
Attend all required meetings, training, and agency events.
Maintain compliance with data entry, record keeping, and documentation within the electronic database and client files.
Complete all reports as required.
Provide services in a manner that is trauma-formed, client-centered, and culturally competent.

Other duties as assigned by the Director of Reentry Programs.

OTHER REQUIREMENTS
Valid California Driver's License and auto insurance.
Ability to pass a San Joaquin County Criminal Background Check
Treat staff, colleagues, and clients with respect and dignity.
Communicate effectively, written and orally.
Physical capacity to lift and carry containers and materials up to 20 pounds (utilizing safe and accepted methods to avoid injury), walk .25 miles on inclement weather and in the dark, and move quickly, in case of an emergency.
