Re-Credentialing
To remain active and compliant within our NEMT network, all providers are required to complete re-credentialing by submitting the documentation outlined below.
Required Documentation
1. Compliance Checks (All Drivers)OIG LEIE and SAM exclusion screenings for all active drivers.
2. Driver CredentialingValid driver's license (current and unexpired)Criminal background check and sex offender registry screeningMVR/DMV record checkTraining verification (minimum: Rules of the Road)Training must include: safety and emergency procedures, HIPAA/privacy, ADA and nondiscrimination, wheelchair securement (if applicable), and Fraud, Waste, and Abuse (FWA)
3. Vehicle Credentialing:Current registration for all company vehicles
4. Certificate of Insurance (COI)Proof of Commercial General Liability and Auto Liability coverage. All active vehicles must be listed by VIN on the policy. Certificate Holder: NationsBenefits, 1700 N University Dr, Plantation, FL 33322
