Job Responsibilities:
The MDS Coordinator ensures the accuracy of our resident assessments and care plan optimization. To achieve this goal, this position's responsibilities include, but are not limited to:
  • Conducting and coordinating the development and completion of resident assessments (MDS) by current rules, regulations, and guidelines
  • Ensuring accurate and timely completion of the MDS process
  • Collaborating with the interdisciplinary team to develop individualized resident care plans
  • Monitoring and documenting resident progress and responses to care plans
  • Participating in the Quality Assurance and Performance Improvement (QAPI) process
  • Communicating with residents, families, and other healthcare providers to coordinate optimal care delivery
  • Staying updated on MDS regulations and guidelines to ensure compliance
