MDS Coordinator/NAC (RN or LPN)

We are seeking a skilled MDS Coordinator/NAC to join our team and play a key role in guiding the Resident Assessment Instrument (RAI) process, ensuring accurate and timely assessments, supporting strong reimbursement outcomes, and helping deliver the highest quality of care to our residents.

What You'll Do

  • Lead and manage the completion of Minimum Data Set (MDS) assessments in compliance with federal and state regulations.

  • Partner closely with the interdisciplinary team to develop meaningful, individualized care plans that reflect each resident's needs and goals.

  • Ensure documentation and coding are accurate, complete, and aligned with reimbursement requirements.

  • Review clinical data to identify trends, strengthen care outcomes, and support continuous quality improvement.

  • Stay current on regulatory updates, MDS best practices, and changes affecting resident assessment and reimbursement.

What We're Looking For

  • A clinically strong, detail-oriented professional with high standards who is collaborative and passionate about reimbursement and quality outcomes.

  • Current Registered Nurse (RN) license in the State of Michigan. Exceptional Licensed Practical Nurses (LPNs) will also be considered.

  • At least two years of experience with MDS 3.0 and the RAI process preferred.

  • RAC-CT certification preferred.

  • Exceptional organizational skills, accuracy, and attention to detail.

  • Strong communication skills with the ability to collaborate effectively across nursing, therapy, social services, dietary, and leadership teams.

  • A proactive, resident-focused professional who takes pride in quality outcomes, compliance, and teamwork.
