Service Coordinator - Assertive Community Treatment Team
Annual Salary: $52,278
Schedule: Monday - Friday, 8:30 am to 5:00 pm
ABOUT US
Since 1971, the Hampton-Newport News Community Services Board has provided the highest quality of behavioral health services to residents of the cities of Hampton and Newport News, Virginia, offering a comprehensive array of programs for both children and adults living with mental illness, developmental disabilities, and/or substance use disorders.
Assertive Community Treatment (ACT) is a team-based program of medical, behavioral health, and rehabilitative professionals who support individuals with severe and persistent mental illness, primarily schizophrenia, who require intensive services to remain stable. The goal of ACT is to reduce symptoms, prevent relapse, improve daily functioning and quality of life, increase community stability, reduce hospitalizations, and lessen the burden on families.
ROLE SUMMARY
As a Service Coordinator on the Assertive Community Treatment (ACT) Team, you help individuals with severe and persistent mental illness access coordinated, community-based supports that promote stability and recovery. Key responsibilities include completing intakes and eligibility determinations; developing, updating, and documenting individualized service plans; coordinating referrals and follow-up with providers; advocating for client access to housing, healthcare, benefits, and other resources; supporting discharge planning; and participating in team case reviews and related proceedings as needed.
Required qualifications:
  • Bachelor's degree in Human Services such as Social Work, Psychology, Counseling.
  • One (1) year of case management/service coordination experience with severe and persistent mental illness.
  • QMHP registration (or QMHP-trainee registration) within 30 days of hire.

YOUR TYPICAL DAY
Your day runs Monday through Friday, 8:30 a.m.-5:00 p.m., balancing planned outreach with time-sensitive needs. You'll connect with clients, families, and community partners through calls, office visits, and home-based contacts, tracking progress toward current goals and adjusting supports as circumstances change. Expect regular coordination touchpoints-scheduling appointments, confirming follow-through, and keeping communication clear across care providers. You'll also complete timely notes and reports, update records after contacts, and bring key updates to team discussions. When urgent situations arise, you'll help provide crisis support and ensure next steps are in place before transitioning back to scheduled work.
