Job description

The Hospice & Palliative Care RN QAPI (Quality Assurance Performance Improvement) Specialist will work at the direction of the Director of Quality, collaboratively with the Directors of Hospice and Palliative Care, appropriate Managers, members of the QAPI Committee to support Hospice and Palliative Care QAPI activities and to meet performance improvement objectives. They are responsible for developing, implementing, maintaining, and evaluating the QAPI Program for Palliative Care, and collaborating on developing, implementing, maintaining, and evaluating the QAPI Program for Hospice. This includes monitoring patient clinical records and providing relevant staff education and coaching. Leads and evaluates performance improvement processes using best practices and established protocols to effect change. Coordinates data collection and reporting needs in accordance with clinical standards and expectations, agency policies and procedures, and applicable laws and regulations. Establishes systems to ensure the integrity of data input and reporting.

Monday - Friday; 8:30am - 5:00pm

4 days onsite; 1 day remote

Based in Larkspur, CA with occasional travel to our Pleasanton, CA and San Mateo, CA Offices

Essential Duties & Responsibilities include (but are not limited to):

  • Develops, implements, maintains, and evaluates an effective data driven QAPI program for Palliative Care services. Coordinates the agenda, reports, presentations, and discussion for palliative care services at QAPI meetings. Performs ongoing monitoring of the QAPI Program.
  • In collaboration with QAPI Specialist - Hospice, develops, implements, maintains, and evaluates an effective data driven QAPI program for Hospice services. Coordinates the agenda, reports, presentations, and discussion for palliative care services at QAPI meetings. Performs ongoing monitoring of the QAPI Program.
  • Completes selected and random documentation review including Start of Care, recerts, and discharges, with attention paid to timely review of EPIC assessments. Conducts concurrent and retrospective chart audits. Oversees accuracy of diagnosis coding and EPIC assessments.
  • Compiles and submits quarterly Quality Control, Performance Improvement and Clinical Record Review reports. Works with Palliative Care leadership to submit required Health Plan data reports.

Education/Experience:

  • Graduate from an accredited school of Nursing. BS in Nursing .
  • A minimum of two years of hospice, Palliative Care or community health experience required.
  • Quality background preferred.

Certificates, Licenses, Registrations:

  • Public Health Certification preferred
  • Current RN license to practice in the state of California. 

Pay Range:
$48.95 - $61.25 hourly
