Job description

POSITION SUMMARY
The Regional QAPI Director for Hospice Division coordinates and implements clinical strategies and provide leadership development for clinical team members on quality analysis, direction, and performance improvement related to clinical care. They provide overall guidance and support for regulatory and compliance matters on delivery, service, and quality outcomes. They will track trends and reports data needed to monitor clinical compliance and agency standing in relation to national benchmarks, outcomes, policy implementation and other types of agency performance standards, and compile quarterly reports. This position will cover our Arizona and California territories. The candidate must be living in Arizona or California. Arizona is highly desired.
REPORTS TO: Vice President of Clinical Services
SUPERVISES: Quality Assurance Team
QUALIFICATIONS:
Education: Graduate of an accredited school of nursing with a bachelorâ€'s in nursing or higher.
Credentials: Current, active RN license within the State/s where the company operates. CHPN Certification preferred. Certification in Hospice and Palliative Nursing desired.
Experience: 5+ years current hospice or palliative experience with 2+ years management experience in nursing, preferably in hospice.
Core Competencies: Knowledge of Hospice operations and federal and state regulations. Knowledge of Nurse Practice Act and acceptable standards of Nursing Practice. Must know methods to comply with the regulations. Must have a working knowledge of PI principles and techniques. Must demonstrate effective interpersonal skills to interact with agency/facility staff, corporate leaders, and others. Computer skills; proficient with Microsoft Office applications. Travel required: able to drive to multiple locations. May require occasional overnight travel. Demonstrated competency in performance of professional nursing functions.
Other: Valid driverâ€'s license and auto insurance.
FUNCTIONS AND RESPONSIBILITIES
1. Utilizes clinical knowledge to coach and mentor clinical leaders to drive clinical excellence and addresses areas of improvement on clinical documentation and patient care.
2. Partners with each program in assigned region to determine needs. Coordinates frequency of site visits to meet the identified needs of each program.
3. Maintains expertise in clinical knowledge of pain and symptom management and end of life care.
4. Ensures training and education are provided at the program level by collaborating with Program Leadership.
5. As needed, provides day to day support via phone, email, or visits to Program Management and colleagues concerning outstanding issues of patient care, appropriateness, and compliance.
6. Provides support via phone or visit as needed for state/federal/accrediting surveys.
7. Develops, in coordination with Program Management, plans of correction in the areas of clinical compliance. Subsequently monitors Program execution of plans of correction.
8. Advises the Corporate leadership regarding the clinical and compliance needs/concerns of each program.
9. Reviews and recommends revisions to policies and procedures in accordance with updated state and accrediting body standards.
10. Maintains absolute confidentiality of agency/facility records including, but not limited to payroll, personnel, patient/client care, business plans, and computer program.
11. Complies with all organizational policies regarding ethical business practices.
12. May participate in governing body oversight to achieve excellence in quality and service.
13. Ensures compliance with clinical and operational policies, procedures, and processes.
14. Collaborates with program and regional leadership to analyze and strategize methods to achieve clinical and operational metrics.
15. Collaborates with program and regional leadership in the selection, on-boarding, development, and retention of new leaders.
16. Supervises/overseas Quality Assurance Team members and/or Patient Care Managers of assigned locations per requirements and as needed.
17. Review and analyze PI/QAPI, Focus Review, UR, and other compliance related data. Leads quarterly QAPI reviews.
18. Collaborate with the Leadership team and agencies to meet the annual clinical objectives.
19. Responsible for monthly multidisciplinary comprehensive chart audits.
20. Responsible for bringing forward issues of concern affecting quality of care in the agencies.
21. Assists to provide orientation to key positions of the hospice Clinical Teams, participates in educational programs throughout the company related to hospice regulations and other clinical issues as assigned.
22. Interprets and translates new agency policies and procedures and assesses compliance to coordinate communication and correspondence with accreditation requirements and other type of regulatory bodies that may apply.
23. Assists with development of plan of correction to survey deficiencies.
24. All other tasks and duties deemed necessary and appropriate.
