Description
StarCare Specialty Health System seeks to hire a Registered Nurse (RN) to serve as the Contracted Services Liaison for the PACE (Program of All-Inclusive Care for the Elderly) Program delivered at Silver Star (a division of StarCare).
The Contracted Services Liaison RN works and has good rapport with the Silver Star Program Director, Health Services Director, Medical Director, Center Manager, nursing team and the Quality Assurance and Performance Improvement (QAPI) Coordinator, to ensure the highest degree of quality care is maintained at all times.
Contracted Services Liaison RN Responsibilities:
  • Ensures the provision of healthcare services to those supported in the Program of All-inclusive Care for the Elderly (PACE).
  • Develops relationships with the staff in each contracted facility to confirm the care, length of service, and therapies are necessary.
  • Makes facility visits to assess participants who reside in a nursing home, assisted living facility or skilled nursing facility, etc.; including weekly review of all participants in post-acute skilled care facilities and communicates with primary care physicians and IDT.
  • Makes weekly visits to nursing homes and bi-weekly nursing visits to assisted living facilities as well as medication administration monitoring.
  • Makes visits to meet with discharge planning personnel from facilities, as appropriate.

The Contracted Services Liaison RN is an integral team member responsible for:
  • Performing annual, semi-annual, and as needed assessments;
  • Maintaining participant files, including daily coordination of care;
  • Ensuring services are in accordance with physician orders;
  • Ensuring orders are signed in a timely manner;
  • Reviewing nursing documentation for appropriateness; and
  • Preparing for audits.
  • Use effective relationship management, coordination of services, resource management, education, patient advocacy, and related interventions to:
  • Promote improved quality of care and/or life,
  • Promote cost effective medical outcomes,
  • Assist in prevention of hospitalization when possible and appropriate,
  • Promote decreased lengths of hospital stays when appropriate,
  • Assist in working with medical and nursing team preventing complications in participant health status that are under the care of Silver Star, and
  • Check IDT decisions, ensuring appropriate levels of care are received by participants.

Skills & Abilities:
  • Must demonstrate outstanding verbal communication, participant-focus and critical thinking skills, excellent judgement, self-motivation, capable of working under pressure in a busy setting, projecting a friendly and professional image, and the ability to work independently and as a team member.
  • Advocates on behalf of the participant and demonstrates accountability in resolving concerns or issues, while maintaining an optimal level of independence for enrollees/participants.

Schedule: This is a full-time/non-exempt position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the program.
  • Provides after-hours on-call medical assistance on a rotating basis, via phone triage or after-hours home visits to participants as needed.

On-Call Pay:
  • $25.00/Day Monday - Friday and $75.00/day for Saturday & Sunday and StarCare Approved Holidays.

Qualifications:
  • Graduation from an accredited school of nursing and currently licensed in the State of Texas by the Board of Nurse Examiners as a Registered Nurse, required.
  • Bachelor of Science in Nursing (BSN), preferred.
  • Four (4) years of full-time experience as a practicing registered nurse, preferred.
  • At least one (1) year of experience with a frail or elderly population, required.
  • Must have up to date immunization status and cleared of communicable diseases.
  • Prefer experience in home health care setting, with emphasis on interdisciplinary approach to treatment.
  • Supervisory experience is also preferred.

Additional Requirements:
  • Skill and proficiency in oral and written communication, strategic planning, and operating a personal computer with Microsoft Office Products (Word, Excel, Access, and email).
  • Must have reliable transportation, liability auto insurance, current Texas driver's license, and be insurable under corporate insurance.
  • Must pass pre-hire criminal background check and drug screen.
