Job description

We are a well-established hospice organization with over 35 years of dedicated service to the South Texas community, recognized for our commitment to compassionate, patient-centered care. We are seeking a Quality Assurance / Performance Improvement (QAPI) Coordinator to lead and support our ongoing efforts to ensure regulatory compliance, clinical excellence, and continuous organizational improvement.

The ideal candidate will bring strong analytical capabilities, regulatory knowledge, and a proactive approach to quality management in a hospice or healthcare setting.


Key Responsibilities

  • Oversee and manage the organization’s Quality Assessment and Performance Improvement (QAPI) program in accordance with CMS Conditions of Participation and state regulatory requirements
  • Monitor, analyze, and report quality metrics to identify trends, risks, and areas for improvement
  • Develop, implement, and evaluate Performance Improvement Projects (PIPs) aimed at enhancing patient care outcomes and operational effectiveness
  • Conduct routine and focused audits of clinical documentation, processes, and compliance standards
  • Collaborate with interdisciplinary teams to develop and implement corrective action plans
  • Prepare and present quality and compliance reports to leadership and governing bodies
  • Ensure timely response to regulatory updates and maintain readiness for surveys and audits
  • Provide education and guidance to staff on quality improvement initiatives and compliance standards


Qualifications

  • Minimum of 2 years of experience in hospice, home health, or healthcare quality assurance
  • Working knowledge of QAPI principles, CMS hospice regulations, and accreditation standards
  • Strong analytical, organizational, and communication skills
  • Demonstrated ability to interpret data and implement effective quality improvement strategies
  • Proficiency with electronic medical records and quality reporting systems


Preferred:

  • Current licensure as an LVN or other relevant healthcare credential
  • Prior experience in hospice quality or compliance leadership
