POSITION SUMMARY:
The Nurse-Family Partnership (NFP) Grant Reporting and Outreach Coordinator (GROC) assures grant stability and fidelity, budget accountability, and is accountable to meet the stat requirements to maintain adherence to the project work plan each fiscal year. This position is responsible for planning, implementing, and monitoring the departmental budget under the guidance of the NFP Administrator and the NFP Director, to ensure coordinated program quality improvement based on program performance data and outcomes. The Grant Reporting and Outreach Coordinator will promote program integrity and data quality by collaborating with the NFP Director and the Data Entry Clerk in navigating the current DCS, running data quality reports, data entry, and coordinating regular CQI efforts. This position will represent Nurse Family Partnership at DHR Health Women's Hospital and assist the NFP Director within the community and actively create connections between community partners to increase leads and referrals for the program. The goal for this position will be achieved by conducting professional public service work in collaboration with and support of the NFP Administrator, NFP Director, and Nurse Home Visitors in the development, coordination, and implementation of the Nurse-Family Partnership home visiting program.
POSITION EDUCATION/ QUALIFICATIONS:
• Valid Driver's license and insured automobile required.
• Computer skills required with knowledge of Microsoft Office suite.
• Effective written and verbal communication skills are required.
• Some evening or weekend work will be required.

Responsibilities:
POSITION RESPONSIBILITIES:
• Oversee program and outreach expenditures ensuring adherence to annual fiscal year allocations with guidance and support of the NFP Director and NFP Administrator.
• Facilitates a meaningful and productive relationship between community partners and Nurse Home Visitors.
• Convene and facilitate community coalitions to assist in community engagement and program sustainability.
• Lead online publications and Social Media Outlets for the NFP program.
• Develop and schedule program activities, such as NFP Client Celebrations, in accordance with specifications and funding limitations.
• Promote program integrity and data quality by collaborating with the NFP Director and Data Entry Clerk in navigating the current DCS, running data quality reports, data entry, and coordinating regular CQI efforts.
• Perform all other related duties as assigned by the Program Director or Program Administrator.
• Ability to travel to support outreach efforts and attend program support conferences.

B. Working Conditions: The individual spends over 95% of his/her time in an air-conditioned environment with varying exposures to noise. There is protection from weather conditions but not necessarily from temperature changes. The position does have low exposure to malodorous, infectious body fluids from patients and some minimal exposure to noxious smells from cleansing agents.
C. Occupational Exposure: This position has minimal to no exposure to blood, body fluids, or tissues and is an OSHA Category III (although situations can be imagined or hypothesized under which anyone, anywhere, might encounter potential exposure to body fluids). Person who performs these duties are not called upon as part of their employment to perform or assist in emergency medical care or first aid or to be potentially exposed in some other way.
