Job description
The Community Coordinator provides program coordination, operational support, partner engagement, and technical assistance for the Healthy Hometowns Health Hub Technical Assistance program. Working closely with the Program Manager, this position supports program planning and implementation, tracks grant deliverables and deadlines, maintains documentation, assists with reporting, and supports compliance with state and federal requirements.
The Coordinator provide support to participating communities, rural partners, and Health Hubs, serving as a primary point of contact building relationships and supporting partners from initial engagement and readiness assessment through implementation. Responsibilities include facilitating planning activities, strengthening partnerships, supporting governance and operational readiness, and providing individualized technical assistance related to Hub-and-Spoke care models, telehealth integration, and other program strategies.
The position helps ensure effective communication and coordination among internal and external partners, monitors program progress and emerging needs, and supports the successful implementation and long‑term sustainability of program goals.
This position is not eligible for University sponsorship for employment authorization now or in the future.
Position Responsibilities: Strategic & Operational Support- Coordinate and administer day‑to‑day operational activities for the Health Hub TA Program, ensuring alignment with program goals, timelines, grant requirements, and deliverables.
- Assist the Program Manager with program planning, scheduling, work plan development, and tracking of milestones, deadlines, risks, outcomes, and implementation progress.
- Track project activities and deliverables to support compliance with contract requirements and University, state, and federal guidelines.
- Analyze program data and prepare progress reports, summaries, and documentation for program leadership, Iowa HHS, and required grant and state reporting.
- Support fiscal oversight by monitoring program accounts, expenditures, and resource allocation; reconciling statements; identifying potential budget variances; and supporting compliance with applicable requirements.
- Advise program leadership on resource needs, budget considerations, operational priorities, risks, and opportunities to improve program efficiency.
- Coordinate program resources and logistics for technical assistance sessions, webinars, meetings, trainings, and stakeholder engagement activities.
- Support effective day‑to‑day communication and coordination with Iowa HHS, Health Hubs, partner organizations, and other program stakeholders.
- Maintain project trackers, calendars, dashboards, work plans, and documentation across program workstreams.
- Document and track action items, decisions, risks, issues, and implementation activities, ensuring appropriate follow‑up and communication.
- Support implementation of clinical and community initiatives, including Hub-and-Spoke models, telehealth, care coordination, and related program activities.
- Coordinate procurement, contract routing, vendor activities, invoice tracking, and other operational needs supporting program implementation.
- Monitor program activities for compliance with grant and contract terms and applicable University, state, and federal requirements.
- Collect, organize, and maintain program, financial, engagement, and implementation data for reporting, evaluation, reimbursement, and compliance purposes.
- Support preparation of grant reports, financial summaries, deliverable documentation, and required submissions.
- Maintain complete, accurate, and audit‑ready records of deliverables, approvals, expenditures, partner interactions, technical assistance activities, and regulatory documentation.
- Conduct readiness assessments to evaluate Health Hub capacity, infrastructure, partnerships, and implementation needs.
- Guide Health Hubs through planning and implementation activities, including governance development, partnership strategies, baseline data collection, workflow design, telehealth readiness, and care coordination.
- Coordinate and provide technical assistance to support Hub-and‑Spoke model development, evidence‑based practices, implementation expectations, and problem‑solving.
- Organize learning collaboratives, trainings, technical assistance sessions, and other activities designed to strengthen Health Hub capacity and long‑term sustainability.
- Provide support between UI Health Care, participating Health Hubs, rural partners, and community stakeholders throughout planning and implementation.
- Build and maintain collaborative partnerships with healthcare organizations, public health agencies, behavioral health providers, EMS, schools, community‑based organizations, and other stakeholders.
- Facilitate stakeholder engagement, kickoff meetings, and ongoing partner communications, including preparation and distribution of agendas, materials, resources, and follow‑up information.
- Develop and coordinate consistent communication regarding program expectations, timelines, requirements, technical assistance, and implementation activities.
- Maintain stakeholder directories, communication records, and engagement documentation.
- Represent UI Health Care in external meetings, training, and collaborative activities and support program visibility and engagement across rural communities.
- Monitor Health Hub progress, partner engagement, program outcomes, and technical assistance effectiveness using program data and established measures.
- Analyze outcomes and engagement data to identify barriers, trends, and opportunities for improvement.
- Evaluate communication, engagement, and technical assistance approaches and recommend adjustments to improve effectiveness and partner outcomes.
- Provide functional coordination across program activities to promote consistency and alignment in technical assistance and program implementation.
- Identify barriers, emerging risks, resource needs, and partner concerns and communicate recommendations to program leadership.
- Provide field‑based insights, program data, and partner feedback to inform statewide planning, outreach strategies, and long‑term program sustainability.
- Support onboarding, coordination, and guidance of staff involved in program and technical assistance activities, as assigned.
Bachelor’s degree in public health, health administration, social work, or related field or an equivalent combination of education and experience is required
1 year of experience supporting programs, projects or grants.
Strong organizational skills with exceptional attention to detail.
Ability to manage multiple priorities and meet deadlines in a fast‑paced environment.
Proficiency in Microsoft Suite (PowerPoint, Word, Excel).
- Willingness to travel statewide.
- Valid driver’s license plus the ability to meet the University of Iowa Fleet Safety program driving standards.
Demonstrated experience working effectively in a welcoming and respectful workplaceenvironment.
Desired qualifications:- 3+ years of experience in community engagement, healthcare coordination, technical assistance and or supporting programs, projects or grants.
- Knowledge of rural health systems and Hub‑and‑Spoke care models preferred.
- Related experience working with state or federal health agencies.
- Knowledge of health care delivery systems or rural health partnerships.
- Experience with data dashboards or project management tools.
