Job description

Summary

The Pediatric Complex Care Coordinator is a Registered Nurse responsible for coordinating comprehensive, familycentered care for medically complex and chronically ill pediatric patients. Ensures timely followup, care plan development, provider collaboration, and connection to specialty services across the continuum of care. Responsibilities include coordinating newborn screening followup processes, supporting diagnostic pathways, facilitating multidisciplinary communication, and improving care quality in accordance with Parkview Health values of Trust, Quality, Flexibility, Teamwork, and Stewardship.

Education

Requires a Diploma in Nursing or an Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred.

Licensure/Certification

Must hold a current Registered Nurse license through the Indiana State Board of Nursing or obtain reciprocal licensure.

Additional certifications may be required depending on patient population needs (e.g., PALS).

Experience

Strong background in pediatric nursing required.

Experience with medically complex pediatric populations, care coordination, case management, or specialty program coordination preferred.

Other Qualifications

Must demonstrate additional training in assessing and planning the biophysical and psychosocial needs of the child and parents. Readily and capable of communicating with parents, as needed.
