Join The Care Team as a Patient Services Coordinator and play a vital role in supporting compassionate hospice care behind the scenes. We are seeking an organized and detail-oriented professional to help coordinate schedules, support clinical operations, and ensure patients, families, and care teams experience seamless communication and exceptional service throughout the hospice journey.

Schedule & Coverage

  • Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
  • Location: In Office - Decatur, Illinois

Key Responsibilities

 • Assign evaluation, admission, and routine visits
 • Process rescheduled, missed, and reassigned visits
 • Run and review scheduling and visit reports
 • Verify visit documentation and clinical notes
 • Ensure unscheduled visits are addressed in a timely manner
 • Document scheduling activity and visit coordination accurately and timely in the EMR HCHB (Homecare Homebase)

Qualifications
 • Healthcare scheduling or coordination experience preferred
 • Strong organizational and multitasking skills
 • Experience using HCHB (Homecare Homebase) preferred

Field Role Requirements
 • Active driver’s license
 • Ability to pass a criminal background check
 • Ability to pass a Motor Vehicle Record (MVR) check
 • Current TB testing in accordance with agency policy
 • Current CPR certification as required for the role
