Location: Must be based within one of the following regions:

  • Dayton

Job Summary/Purpose - The Practice Manager (PM), Palliative Care provides oversight of OHI's Palliative Care Program, to include referrals, scheduling, documentation, billing, coding, quality reporting and other operations. The PM is supported by and supervises the Care Team Coordinators (CTCs). The PM ensures operational alignment with contracted and non-contracted community partners to include hospitals, facilities and provider groups. The PM will collaborate with Pure Healthcare in the support of population health contracts, navigating processes to achieve successful outcome measurements.

Qualifications-
  • Minimum of five years medical practice management experience.
  • Bachelor's Degree in business administration, healthcare administration or related field
  • Excellent written and verbal communication skills
  • Computer skills sufficient to properly document services and analyze data including Microsoft Office 365, Teams, Power Point
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage
Key Responsibilities
  • Collaborate across the organization, and uniquely with provider staff, to maintain quality driven, outcome measured Palliative Care in all settings.
  • Collaborate with physician leaders to provide evidence-based care in accordance with the organization's mission, vision and strategic objectives.
  • Contribute ideas and solutions directed toward an evolving model of Palliative Care
  • Understand, teach and reinforce protocols, systems, and processes.
