Overview

Under general direction performs activities and duties related to recruitment, in-service education, preadmission evaluation, and admission of candidates who meet program criteria and will benefit from program services.

Responsibilities
  • 1. Performs On-Site and electronic Preadmission Screening and Clinical Assessment and Provides Recommendations for Appropriate Level of Care.
  • 2. Educates Potential Patients and/or Significant Other, Referral Sources, Hospital Personnel, And External Case Managers as To Rehabilitation Services and Benefits.
  • 3. Works as The Liaison Case Manager with Internal and External Case Management Teams to Assist with Precertification for Admission And facilitates Approval for Patient Admission.
  • 4. Maintains Thorough Knowledge Of L&M Rehabilitation Process, Policies, Services, Acuity Trends, Outcome Data and Provides Information Necessary to Facilitate and Promote Referrals and Subsequent Admissions.
  • 5. Collaborates with The Admitting Physiatrist and Nurse Manager, Or Designee, To Facilitate Timely Responses to All Stake Holders and Timely Admission for Patients Meeting Program Criteria.
  • 6. Develops A Relationship with Referral Sources.
  • 7. In Collaboration with Physiatrists and Nurse Manager Develops Annual Goals Related to Impact Of Role On Volume Growth.
  • 8. Provides Tours for Public and Professional Education.
  • 9. Meets Performance Expectations for Customer Service, Teamwork, Resource Utilization, And Staff and Self Development As Outlined In Performance Review.
  • 10. The PPS Coordinator aspect of this role is responsible for data collection of each patient, prioritization of data, and input of data into the UDSMR system. In addition, the PPS coordinator is responsible for education of staff for compliance with regulatory CMS documentation.
  • 11. Meets Performance Expectations For Customer Service, Teamwork, Resource Utilization, And Staff And Self Development As Outlined In Performance Review.
  • 12. Performs Other Duties As Assigned Or Directed To Ensure Smooth Operation Of The Department/Unit.

Qualifications
EDUCATION
Graduate of an accredited school of Nursing. Bachelor of Science in Nursing (BSN) preferred.
EXPERIENCE
Minimum of two years Med/surg nursing experience required. Clinical Rehab experience preferred. Case management experience preferred.
LICENSURE
Must have and maintain current license as a Registered Nurse in the state of Connecticut.
SPECIAL SKILLS
Demonstrated Ability in Time Management, Decision Making, Communication and Computer Use. Demonstrated Excellence in Assessment Skills. Knowledge Of Various Rehabilitation Settings, Medicare Regulations for Rehabilitation and Other Third-Party Payer Reimbursement Will Be Essential to Have Current Knowledge or Ability to Learn Quickly.
