PHC/PAS Home Care and Field Supervisor

Angel Home Healthcare Services is an award-winning and leading home health provider serving Dallas and surrounding counties. We are an agency focused on delivering compassionate, high-quality care to patients and clients in their homes or places of residence, including non-medical personal assistance, skilled nursing, and rehabilitation. Agency is seeking an experienced Primary Home Care and Field Supervisor for our PAS/ PHC Department. Our caregivers provide high quality and timely care, as we focus on each unique patient to deliver the appropriate care and support to our patients and their families.
The Primary Home Care Supervisor will work with other Primary Home Care Supervisors in the team and coordinate with other agency staff to provide support of personal attendant services (PAS) provided to clients, and in accordance with state regulations/company policy. This position will work in a fast pace environment and seek to innovate creative solutions for in home care. If you feel you are a good fit, we encourage you to apply!

Qualifications:

  • Must have at least Two (2) years of experience as a PHC/PAS Supervisor.
  • One year of experience and/or training in the care of individuals with functional limitations
  • Excellent communication and interpersonal skills to interact with clients, staff, and agency partners.
  • Supervisory experience in a home health setting
  • Computer Literacy
  • Advanced knowledge of Vesta EVV / Datalogic software (required)
  • Working knowledge of electronic claims submissions for managed care organizations
  • High school diploma or GED (required)
  • Some College to College Degree (preferred)

Job Responsibilities will include (but are not limited to):

  • Supervise all attendants and caregivers
  • Responsible to perform Competency Evaluation to all attendants.
  • Responsible for conducting recruiting and hiring of new attendants in-office and or in-house and complete the orientation forms
  • Applicant will maintain and audit client files, perform annual and assist with supervisory visits, perform orientation and supervision to providers and clients as required by agency policy.
  • Maintain and supervise all aspects of client quality care as per ISP.
  • Communicate with case managers, practitioners, and others partners to facilitate client services and to ensure client continuity of care.
  • Perform all assigned tasks in compliance with the agencys policies and procedures.
  • Perform all assigned tasks of field visits including Supervisory Visits, Admissions, Attendant Orientations, etc.
  • Responsible to ensure that all attendants are performing all assigned, allowable tasks & schedule according to clients service plan.
  • Address and report any complains voiced by client or attendant to Alt. Administrator /Administrator.
  • Perform all assigned tasks in compliance with the agencys policies and procedures.
  • Assist with performing and monitoring daily maintenance of electronic visit verification (EVV) data for all PAS attendants in Vesta EVV/Datalogic Software.
  • Assist in Monitoring proper usage of EVV Reason Codes, client visit entries, and runs checks and balances prior to processing of payroll/submission of claims
  • Special projects and other tasks as assigned by the agency's Administrator.
  • Maintain outstanding relationship with clients, other employees and attendants.
