Looking for a PER DIEM Speech Therapist for a Skilled Nursing Facility in Rochester, NY

Work Location : Wesley Gardens - 3 Upton Park, Rochester, NY 14607, USA

Job Responsibilities:

  1. Provide quality SLP services, including evaluating patients, screening, initiating referrals, obtaining data and follow through with plan of care.
  2. Completes timely documentation including progress notes, screen reports, daily notes, and discharge summaries.
  3. Knowledgeable in short term and long term SLP care.
  4. Instruct patients as well as family members and staff in goals, progress, and appropriate therapeutic techniques.
  5. Ensures compliance with protocols and regulations.

Experience and Qualifications:

  1. Valid NYS License & certificate of clinical competency
  2. Preferred knowledge of Rehab Optima
  3. Experience with cognitive and dysphagia rehabilitation within a skilled nursing facility.
