Per Diem Speech-Language Pathologist (SLP)

Location: Lilac Manor – Rochester, NY

Position: Per Diem

Compensation: $58–$62 per hour

Lilac is seeking a dedicated and experienced Per Diem Speech-Language Pathologist (SLP) to join our rehabilitation team. This role offers the opportunity to provide meaningful, patient-centered care in a supportive skilled nursing environment while working collaboratively with an experienced interdisciplinary team.

Responsibilities

Evaluate patients and provide appropriate speech-language pathology services based on individual clinical needs.

Conduct screenings, initiate referrals, gather relevant clinical information, and develop individualized plans of care.

Provide treatment for communication, cognitive, and swallowing disorders.

Complete timely and accurate documentation, including evaluations, progress notes, daily treatment notes, and discharge summaries.

Educate patients, families, and facility staff regarding treatment goals, progress, and therapeutic techniques.

Collaborate with nursing, therapy, medical, and interdisciplinary team members to support positive patient outcomes.

Maintain compliance with facility policies, professional standards, and applicable state and federal regulations.

Qualifications

Valid New York State Speech-Language Pathologist license.

Certificate of Clinical Competence (CCC) required.

Experience with cognitive and dysphagia rehabilitation in a skilled nursing facility setting preferred.

Experience with Rehab Optima preferred.

Strong clinical, communication, organizational, and documentation skills.
