Job description

) Alliance Health at Baldwinville is a 94-bed long term/rehab care facility located in Baldwinville, MA. We are looking for a per diem Speech Language Pathologist to join our Rehab team.
Responsibilities Include:
Evaluates and screens all resident referrals for the Rehabilitation Department for Speech-Language Therapy.
Interprets evaluations and may consult with other rehab team members to assess goals and plan of treatment. Works in conjunction with the Interdisciplinary Team to develop the overall Resident Care Plan.
Establishes a plan of care and treatment in cooperation with and under the prescription of the attending physician.
Treats or delegates treatment of all resident's with orders for Speech-Language Therapy.
Instructs resident's, families, other staff in the resident's treatment regime. Provides written materials as indicated.
