Job description

Job Summary

The Care Coordinator Assistant is responsible for performing clerical and secretarial tasks given by Licensed Care Coordinators to properly plan and implement patient care. The CCA must maintain excellent relationships with the office and field staff, communicate with doctor offices and insurance companies in a high proficiency, have a problem-solving attitude. The employee must submit reports before the end of the day and report to case managers, operations managers, and the DON. This position Is a great opportunity for an individual who pursues a career in healthcare administration or in nursing.

Responsibilities

  • The Care Coordinator Assistant provides clerical, secretarial, and administrative support for the Care Coordination department
  • This position may function as the point of contact for the department, and is responsible for the professional and timely response to calls, messages, and the forwarding of communications to the responsible party
  • The Care Coordinator Assistant may be assigned other duties within the scope of the role, including, but not limited to, scheduling, faxing, sorting, editing, and filing records and forms
  • The Care Coordination Assistant performs these duties according to applicable hospital policies, standards of practice, and Federal and State regulations
  • The Care Coordination Assistant prioritizes and forwards communications to the appropriate Care Coordinator and documents in the designated EMR system.
  • This coordination ensures a plan of care for patients in all stages of health needs
