A Path of Care Hospice is part of a partnership of home health, hospice, and private home services agencies. The mission is to relieve pain, improve comfort, and help patients maintain dignity and independence in their homes. Care teams work closely with physicians to create individualized plans of care, guided by the organization's core values of credibility, accountability, responsiveness, and empathy.

What You'll Do as a Hospice Patient Care Volunteer

Volunteers extend the reach of hospice staff by offering companionship and practical support that helps patients remain safely and comfortably at home. Responsibilities may include:
