Position Summary
The Social Worker assists the Social Work Manager in planning and carrying out social work programs for Members and families of the Home.
Support Culture of Excellence through active role with assigned neighborhood initiatives.
Essential Job Functions:
  • Conducts interviews with Members, families and/or staff for purposes of MDS completion. Documents in the electronic medical records.
  • Assists in developing Patient/Member Care Plans. Completes care plan in the electronic medical record.
  • Assesses Members' personal, financial and social needs.
  • Interprets the care plan to Members and families.
  • Records progress notes in electronic medical record concerning services rendered in relation to a Member's personal and social problems and/or needs.
  • Participates in the education and training of Social Work students.
  • Assists in discharge planning under the supervision of the Director of Social Work. Coordinates community-based services and/or transfer requirements to another facility.
  • Interprets the psychosocial functioning of Members to the interdisciplinary care team. Provides emotional support to Members and families.
  • Utilizes community resources where appropriate.
  • Keeps Director of Social Work/Social Work Manager informed of the emotional, social and/or financial status of Members.
  • Assists Director of Social Work/Social Work Manager in planning programs which would enhance the social work department.
  • Participates in In-Service Education and Safety Programs. May attend meetings and workshops to improve job skills.
  • Coordinates family/staff conferences relating to Member care issues.
  • Coordinates and participates in hospice evaluations and enrollments.
  • Coordinates and assists with Member room transfers.

Qualifications of the Position:
  • Education: Bachelor’s degree in Social Work or a related field.
  • Experience:
  • Skills: Thorough knowledge of the case work process. Knowledgeable in the area of gerontology. Excellent oral and written communication skills. Must be neat in appearance.
  • Licenses / Certifications:

Expectations of the Position:
  • Dependable, report to work on-time for every schedule shift (ready to work at beginning - start of shift) and work full schedule shifts unless physically unable or pre-approved Paid Time Off (PTO) which includes personal and vacation.
  • Willing and able to work overtime and weekends as required by the position and/or approved by management.
  • Have reliable transportation to and from work.
  • Must be clean and neat in appearance and have a “friendly” service-oriented, cooperative, demeanor and supports Culture of Excellence initiatives by interactions with co-workers and/or Members.
  • Abide by work-related / job procedures and organization policies; (refer to the Employee Handbook).

Conditions of Employment:
  • Work is performed in a clean, well lit, “Home-Like,” nursing home setting.
  • Employee must be able to perform essential job functions (reasonable accommodation may apply).
  • OSHA Bloodborne Pathogens Level 1 position – refer to Administrative Policy #39 for details.

Physical Requirements:
  • Refer to Physical Requirements form.
  • Must meet the general health requirements set forth by Friendly Home.
  • May be requested to assist in the evacuation of Members or perform other procedures to protect the safety of Members, visitors and team members during emergency situations.

PHYSICAL EFFORT REQUIREMENTS:
  • Occasional lifting/carrying of 0-10 pounds throughout day
  • Transporting members requires pushing/pulling of 0-200 pounds occasionally throughout the week
  • Writing/typing tasks require repetitive use of both hands frequently throughout the day
  • Sitting is required 50% of the day
  • Standing is required 25% of the day
  • Walking is required 25% of the day
  • Job requires bending and crouching occasionally throughout the day
