Overview
License Behavioral Health Counselor- CMG Resident Program, West Mall Medical Park
PartTime, 48 Hours Per Pay Period, Day Shift
Covenant Medical Group Overview:
Covenant Medical Group is the employed and managed medical practice organization of Covenant Health, providing comprehensive care across East Tennessee. With more than 300 physicians and advanced practice providers in 20 communities, our team delivers expertise across a broad spectrum of specialties from primary care and walk-in clinics to preventive medicine and advanced surgical and subspecialty services. We are committed to offering coordinated, patient-centered care that spans the continuum of health needs, ensuring access to exceptional providers close to home.
Position Summary:
Provides professional social work and counseling services to designated patient and resident population within assigned Covenant Medical Group clinics as well as assisting in referrals to social and community support services. Services include interviewing, assessment, planning, intervention, evaluation, case management, mediation, supportive counseling, information and referral, problem solving, client education, and advocacy. This position has documented advance practice skills via certification and experience.
Responsibilities
  • Assists patients/family/significant other with physical, emotional, and social concerns that arise during the episode of care.
  • Provides treatment/discharge planning services to patients.
  • Completes required documentation including, treatment plans, safety plans, and progress notes.
  • Completes initial assessment and continued documentation to obtain pertinent background/social/ financial data. Documents social determinants of health as indicated in medical record.
  • Connects patients/family/significant other with community resources to aid in their treatment and recovery. Coordinates care with community providers as needed.
  • Acts as facility liaison with adult and children protective services as indicated.
  • Cooperates with facility staff and physicians, including resident physicians and keeps them informed of progress with patients via verbal communication and chart documentation in medical record.
  • Exhibits the skills of empathy, communication, organization, critical thinking, active listening, and patience.
  • Participate in warm handoffs with faculty and residents within clinic, modeling collaborative behavioral care.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
  • Performs other duties as assigned.

Qualifications
Minimum Education:
Master's degree from accredited graduate social work or counseling program.
Minimum Experience:
Two (2) years of experience or related field work training from an accredited graduate social work or counseling program.
Licensure Requirement:
Must be licensed as LCSW, LPC, or LMFT.
