Job description

We are looking for a few fantastic, caring and reliable nurses! The Moments of Lakeville is home to the most outstanding staff and residents. With our 92 suite memory care and enhanced assisted living on Lakeville Campus we continue to bring the highest level of care, love and support to our residents. You are part of a muti nursing team, with 24/7 on site nursing coverage. We are currently seeking Evening (part time and full time) and Weekend Positions!
LPN Position Specifications:
  • This position will ensure that the health and safety of all residents are met.
  • Lead and direct your care team along with actively performing cares of each resident as needed.
  • The LPN will collaborate with residents, families, physicians and Moments staff to create a person-centered plan of care that enhances the life and care of the resident.
  • Must have innate passion for serving seniors and those living with dementia.
  • Must possess the skills to communicate effectively with residents, family members, physicians, all vendors and RN Supervisors.
  • Must be able to prioritize and organize work effectively and efficiently.
  • Commitment to a compassionate care philosophy and whole person approach.
  • Excellent judgment and decision-making abilities and problem-solving skills.
  • Experience with EHR/EMR (electronic health/medical records), ideally Point Click Care.
  • Support the orientation and education requirements of staff.
  • Ability to work with a team to develop person-directed care and outstanding sensory and cognitive programming, in a culture of compassion, respect and dignity.

Other Requirements:
Must be licensed with the State of Minnesota as an LPN and must maintain current and unencumbered licensure.'
  • Must complete a criminal background check from Department of Human Services and may not be on the OIG exclusions list.
  • Must be screened for TB.
