Excellent communication, active listening, and empathy are critical soft skills for engaging with clients and supporting team coordination. These competencies ensure efficient processing of client information, accurate record-keeping, and a positive experience for both clients and staff.

Job description

Job Title: Weekend Home Health Intake Coordinator | Admissions & Referral Management
Compensation: $24-$28 per hour
Location: Golden Valley, Minnesota
Schedule: Part-Time | Friday-Sunday or Friday-Monday | Every Weekend Preferred (Every Other Weekend Considered)
Training: Monday-Friday | 8:00 a.m.-5:00 p.m. for the first several weeks

To Stand Out

Email Elena Ehrlich at Elena.Ehrlich@hhcare.net with your resume and tell us about your home health intake, admissions, scheduling, insurance verification, or referral coordination experience. We're especially interested in candidates who understand what it takes to keep referrals moving quickly and accurately in a busy home health environment.

Be the Person Who Keeps Home Health Moving Through the Weekend.

Referrals come in. Patients need to be admitted. Clinicians need to be scheduled. Hospitals and referral partners need answers.

And it all still needs to happen on the weekend.

Home Health Care, Inc. is looking for a Weekend Intake Coordinator who enjoys being at the center of a fast-moving healthcare operation. You'll help take new referrals from "we have a patient who needs care" to "we have a clinician ready to see them."

If you're organized, responsive, comfortable juggling multiple priorities, and enjoy solving problems before they become problems, you could be exactly who we're looking for.

Why This Schedule Works

$24-$28 per hour

• Choose a consistent Friday-Sunday or Friday-Monday schedule

Every weekend preferred, with every other weekend considered

• Build a predictable schedule around fewer days of the week

• Become a key member of our admissions and clinical operations team

• Work directly with referral sources, clinicians, patients, families, and healthcare partners

• Comprehensive paid training with our weekday team

• Join an established, locally owned home health organization with 30+ years of experience

You'll Own the Weekend Intake Process

This isn't simply answering phones or entering information.

You'll help manage the moving pieces that determine how quickly a new patient can begin receiving care.

Responsibilities include:

Review and process incoming home health referrals while determining whether required information is complete

• Communicate with hospitals, clinics, physicians, referral partners, patients, and families to gather missing information

Verify insurance and payer eligibility and help identify potential reimbursement requirements

• Coordinate nursing, therapy, and other disciplines based on physician orders and patient needs

Schedule Start of Care and other required visits with the appropriate clinical team members

• Accurately enter and maintain patient information within the EMR

• Track outstanding documentation and follow up with referral sources when additional information is needed

• Communicate new admissions and important changes to clinicians and internal team members

• Manage multiple referrals simultaneously while determining what needs immediate attention

• Provide responsive, professional service to referral partners and families throughout the intake process

You'll Thrive Here If You...

• Like being busy and having multiple moving pieces to coordinate

• Are naturally organized and notice when important details are missing

• Can prioritize what needs to happen now versus what can wait

• Communicate confidently with healthcare professionals, patients, and families

• Enjoy solving scheduling and logistical challenges

• Understand that speed matters, but accuracy matters just as much

• Can work independently while knowing when to escalate a clinical or operational concern

What We're Looking For

Required

• Previous healthcare office, coordination, scheduling, admissions, or similar experience

• Strong computer and EMR skills

• Excellent written and verbal communication

• Strong attention to detail and follow-through

• Ability to manage changing priorities in a fast-paced environment

• Availability to complete initial Monday-Friday, 8:00 a.m.-5:00 p.m. training

Highly Preferred

Home health intake or admissions experience

• Home health scheduling or referral coordination experience

• Insurance verification and payer knowledge

• Familiarity with Medicare-certified home health

• Experience working with hospitals, clinics, physician offices, or healthcare referral sources
