Job description

The Home Health RN Case Manager is responsible to plan, organize and direct the Lifespark services while ensuring compliance with regulatory requirements and Lifespark best practices including but not limited to completing, starts of care, care plan development and ongoing support, client satisfaction, caregiver supervision and engagement and on-boarding. The professional nurse builds from the resources of the community & care team to plan and direct services to meet the needs of individuals and families within their homes and communities.


Position: RN Case Manager for Home Health

Salary Range: $88,000-$90,000 annually, depending on experience.

Service Area: Southeast Metro (coverage area: Hastings, Afton, Farmington, Inver Grove Heights, Cottage Grove, South St. Paul, West St. Paul, St. Paul Park, Rosemount, Eagan, Woodbury & surrounding). Plus, mileage reimbursement of 72.5 cents per mile.

Schedule: Monday – Friday 8:00am-5:00pm (with rotation every 6th weekend and 2 holidays/year and on-call as needed)

*Evening RN on-call shift PRN and is paid $125 then PPV for visits completed: SOC $145, ROC $115, NSG Eval, Recert, Oasis DC $85 and SN Visit $70

Position Functions & Responsibilities:

  • Completes an initial assessment of client and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Regularly re-evaluates client nursing needs.
  • Initiates the plan of care and makes necessary revisions as client status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the client and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct client care as defined in the State Nurse Practice Act.
  • Counsels the client and family in meeting nursing and related needs.
  • Provides health care instructions to the client as appropriate per assessment and plan of care.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.
  • Acts as Case Manager when assigned by Clinical Manager and assumes responsibility to coordinate client care for assigned caseload.
  • Lead by example in establishing rapport with clients, clinicians, partners, and the community.
  • Ensure positive/professional communication within the team
  • Customer service resolution; escalate concerns
  • Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.
  • Communicates with the physician regarding the client’s needs and reports any changes in the client’s condition; obtains/receives physician’s orders as required.
  • Communicates with community health related persons to coordinate the care plan.
  • Timely completion of all documentation as it relates to the care plan by 10am the next day
  • Understands and complies with all the rules and regulations that govern the responsibilities of a registered nurse
  • Completes orientation and annual training requirements as assigned
  • Participates in Lifespark’s Event Management process including initiating reports, escalation of events and investigations
  • Promotes safety in the workplace through real time coaching and participation in safety awareness training
  • Instructs, supervises, and evaluates home health aide care provided every two (2) weeks
  • Manages client supplies assuring appropriate DME provider is utilized.
  • Schedules and keeps member appointments
  • Organizes travel efficiently and documents mileage accurately within EMR
  • Other duties as assigned

Home Health RN Case Manager Qualifications:

  • 2 years’ RN experience in health care (preferably home care or hospice)
  • Medicare Skilled Nursing experience and basic understanding of OASIS strongly preferred
  • Experience in customer service/direct member contact field preferred
  • Experience supervising caregivers
  • Knowledge of Medicare Home Health & Hospice regulations
  • Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practice.
  • Graduate of an accredited school of nursing
  • Current Minnesota Nursing License
  • Must pass a MVR check based on Lifespark company policy
