You'll specialize in one of the most important moments in a patient's home health experience: the beginning.

Your job is to understand the complete clinical picture and make sure the patient starts care on the right path.

You'll evaluate their condition, medications, safety, support system, functional needs, and overall home environment. You'll determine what needs immediate attention and coordinate with the interdisciplinary team to make sure the appropriate services are put into place.

Then you'll move on to your next admission.

What You'll Be Responsible For

  • Complete Start of Care admissions for new home health patients.
  • Perform comprehensive skilled nursing assessments.
  • Complete accurate and timely OASIS documentation.
  • Review medications, diagnoses, safety concerns, and disease-management needs.
  • Establish individualized plans of care.
  • Educate patients and caregivers on their care at home.
  • Identify additional nursing, therapy, social work, or other service needs.
  • Coordinate with physicians and interdisciplinary clinicians.
  • Maintain accurate and compliant electronic documentation.

This Role Is Designed For An Experienced Home Health Nurse

We're specifically looking for someone who understands the complexities of home health admissions and can confidently step into a patient's home and take ownership of the assessment.

Required Qualifications

  • Active Minnesota RN license.
  • Current CPR certification.
  • Home health nursing experience required.
  • OASIS experience required.
  • Start of Care or admission experience strongly preferred.
  • Strong clinical assessment and documentation abilities.
  • Confidence making independent clinical decisions in the field.
  • Valid driver's license, active insurance, and reliable transportation.
