Job description

Position Overview:

We are currently seeking a skilled and compassionate Registered Nurse (RN) to join our home health team. As an Admission RN, you will play a vital role in delivering personalized healthcare services to our patients in the comfort of their own homes. You will work closely with a team of dedicated professionals to provide exceptional medical care, promote wellness, and ensure the highest level of patient satisfaction.

This position is a weekend schedule.  The 3-day schedule can be a 10-hour shift that classifies as full time. If 8-hour shifts are desired part time can be discussed.

Responsibilities:

  • Work alongside Case Managers to initiate individualized care plans for patients.
  • Administer medication, perform wound care, and provide skilled nursing services according to physician orders.
  • Set the tone for the patient’s home health experience.  Review consents and assess for urgent patient needs.
  • Educate patients and their families on disease management, treatment plans, and self-care techniques.
  • Document patient information accurately and in a timely manner using electronic medical records (EMR) systems.
  • Coordinate care with other healthcare professionals, including physicians, therapists, and social workers.
  • Adhere to all relevant legal, ethical, and professional standards, ensuring patient confidentiality and privacy.

Qualifications:

  • Valid and active California Licensed Registered Nurse (RN) license.
  • Minimum of 1 year of experience in home health, acute care, or a related healthcare setting.
  • Strong clinical skills, including wound care, catheter care, IV blood draw, medication administration, and patient assessment.
  • Excellent communication and interpersonal skills, with the ability to establish rapport with patients and their families.
  • Proficiency in using electronic medical records (EMR) systems.
