Job description

Physical Therapist - Part-time/PRN- Broward County

In Home Rehab Group provides in-home therapy services to older adults throughout South Florida. We work with Medicare Part A and Part B patients and partner with multiple home health agencies, allowing our therapists to access consistent opportunities through one company instead of having to contract with several agencies on their own.

We are looking for an experienced, dependable, and compassionate Physical Therapist to join our growing team in Broward County. This is a great opportunity for a PT who enjoys treating patients one-on-one in the home setting, wants flexibility, and is looking to add consistent visits to their schedule.

At In Home Rehab of Florida, we do the heavy lifting of finding contracts and building agency relationships so you can focus on what you do best: treating patients. Our therapists benefit from multiple avenues of work, flexible scheduling, competitive compensation, and the support of a team that understands home health.

This position is ideal for a Physical Therapist who is already making home visits and would like to take on additional work, or for a therapist looking for a flexible model outside of the traditional clinic setting.

Responsibilities:

Complete physical therapy evaluations, re-evaluations, progress notes, and discharge visits as appropriate

Treat patients in their homes throughout Broward County

Create individualized treatment plans based on each patient’s needs and goals

Provide skilled, one-on-one care focused on improving strength, balance, mobility, safety, and independence

Supervise and communicate with PTAs as needed

Engage family members and caregivers when appropriate

Provide patient education, home exercise program instruction, and DME recommendations

Complete timely and accurate documentation through EMR

Follow Medicare and home health documentation guidelines

Maintain frequent communication with the office team, patient representatives, and other members of the care team

Requirements:

Active Florida Physical Therapist license

Eligibility to work in the United States

Prior home health or geriatric experience preferred

Strong interpersonal and time management skills

Ability to complete evaluations and documentation accurately and on time

Ability to treat patients independently in the home setting

Reliable transportation
