Occupational Therapist - In-Home Treatment

Job Summary:

In Home Rehab Group, a leading provider of outpatient physical therapy and occupational therapy services, is seeking a highly skilled and compassionate Occupational Therapist to join our team. As part of our In-Home Treatment program, you will have the opportunity to provide personalized occupational therapy services to patients in the convenience of their own homes. Our team is dedicated to delivering exceptional care and improving the lives of our patients.

Responsibilities:

  • Provide occupational therapy services to patients in their homes, focusing on functional skills and daily living activities

  • Develop and implement individualized treatment plans, in collaboration with patients, families, and healthcare providers

  • Conduct thorough assessments and evaluations to identify patients' needs and goals

  • Utilize a variety of treatment techniques and strategies to promote patient progress and independence

  • Maintain accurate and timely documentation of patient progress and treatment plans

  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination

  • Participate in team meetings and professional development activities to stay current with best practices and industry developments
