Job Summary
The Licensed Practical Nurse (LPN) provides skilled nursing care under the direct supervision of a Registered Nurse. This role involves performing selected nursing actions in curative, rehabilitative, palliative, or preventive care while ensuring patient safety and comfort. The LPN is responsible for making informed decisions based on their nursing experience and educational preparation.
Key Responsibilities
  • Assist the Registered Nurse in implementing and carrying out the plan of care.
  • Perform specialized procedures under the supervision of the Registered Nurse and according to physician orders.
  • Participate in the preparation, implementation, and education of patient care plans.
  • Recognize and address the effects of social and economic challenges on patients, providing emotional and physical comfort.
  • Educate patients on appropriate self-care techniques.
  • Prepare clinical and progress notes (excluding admissions).
  • Assist in patient rehabilitation according to the medical care plan by encouraging patient independence and self-capabilities.
  • Actively participate in nursing organizations and in-service programs.
  • Carry out treatments and procedures as assigned.
  • Perform other duties as required.
Qualifications
  • Graduation from an accredited School of Practical Nursing.
  • Active Licensed Practical Nurse (LPN) license in Pennsylvania.
  • At least one year of experience in a Home Health Agency or hospital setting (Home Health experience preferred).
  • Current CPR certification
