Job description

Job Summary:
The Care Coordinator at Generations Home Care is a vital member of our healthcare team, responsible for coordinating and managing the care of our clients in Kitsap County, Washington. This is a full-time, hourly position that requires excellent organizational skills, attention to detail, and compassion. The Care Coordinator will report to the Office Manager and will work closely with other healthcare professionals to ensure the best possible care for our clients.
Compensation & Benefits:
The Care Coordinator position offers a competitive hourly rate of $25- $30 per hour, paid semimonthly. In addition, we offer a comprehensive benefits package that includes health insurance, paid time off, dental/vision, supplemental benefits, 401k, and opportunities for professional development and growth within the company.
Responsibilities:
• Coordinate and schedule care services for clients based on their individual needs and preferences
• Work closely with the Care Coordination Team to create and update care plans for clients
• Communicate regularly with clients and their families to assess their needs and provide support and guidance
• Maintain accurate records of client care, progress, and any changes in their condition
• Collaborate with other healthcare professionals, such as nurses and therapists, to ensure coordinated and comprehensive care for clients
• Assist with recruiting, training, and supervising care staff
• Participate in team meetings and provide updates on client care and progress
• Handle any client or staff concerns or issues promptly and professionally
• Stay up-to-date on industry trends and best practices in home care services
Requirements:
• High school diploma or equivalent, college degree preferred
• Preferred 2 years of experience in home care or a related healthcare field
• Strong organizational and time-management skills
• Excellent communication and interpersonal skills
• Compassionate, patient, and empathetic towards clients and their families
• Ability to work independently and as part of a team
• Ability to learn new technology and work with scheduling software (Wellsky)
• Valid driver's license and reliable transportation
• Able to pass a background check and drug screening
