Job description

8-4:30, M-F

Schedule: Full-time (40 hours/week), Monday-Friday 7:am-4:15pm.

The Treatment Coordinator serves as the initial point of contact for patients, families, and the public, ensuring they are greeted and assisted in a professional and supportive manner. This role plays a critical part in supporting a positive patient experience by coordinating patient admissions and transitions across levels of care. 

Additionally, the Treatment Coordinator provides administrative support to treatment teams and ensures the timely distribution and completion of treatment plan assessments and patient engagement surveys 

Essential duties and responsibilities: 

  • Admissions and Patient Transitions: 
  • Sends welcome emails or other information to patients/families 
  • Identifies referrals for varying levels of care/treatment and coordinate with treatment team and providers to secure these services for patients 
  • Coordinates discharge and/or transfers to other treatment facilities 
  • Assisting with patient admissions: admitting the patient in medical records system, adding treatment team members, helping patients register in the portal, scanning admission documents, issuing admission day schedules, uploading appropriate forms, folder prep, print schedules 
  • Administrative Duties: 
  • Greeting/assisting visitors entering the facility 
  •  Obtaining signed confidentiality statements, issues vendor badges (if needed) and informs appropriate staff of arrival 
  • Answers phones and directs calls to appropriate person 
  • Maintaining supply of patient/family binders and discharge packets 
  • Scheduling meeting spaces 
  • Ensuring staff breakrooms are stocked, lobby and common areas are tidy 
  • Overseeing of incoming and outgoing mail/packages 
  • Coordinating transportation 
  • Submits daily attendance gathered from sign in/out logs or midnight census 
  • Program Support: 
  • Coordinates outside medical appointments for patients as directed by medical or nursing staff 

Qualifications: 

  • High school diploma or GED (required) 
  • Bachelor's degree in healthcare, business, or related field (preferred)
