Job description

All Care Hospice is currently seeking a compassionate, organized, and dedicated Licensed Vocational Nurse (LVN) to join our team as a Hospice Care Coordinator in our Glendale office.

Position Requirements:

  • Current California LVN License (required).

  • Hospice Care Coordinator experience preferred.

  • Strong communication, organization, and time management skills.

  • Ability to coordinate patient care with physicians, nurses, and interdisciplinary team members.

  • Comfortable working in a fast-paced office environment.

  • Willing to make patient visits in the field when needed.

  • Knowledge of hospice regulations and documentation is a plus.


Schedule:

  • Full-Time.

  • Monday through Friday.
