Join our team and make a positive impact on the lives of others! We are looking for a Regional Nurse Manager of Clinical Operations for our Albany Team.
This is a hybrid role from our Albany, NY office.
Department & Position Overview:
The Regional Nurse Manager provides professional nursing care and oversees all clinical operations for the assigned region and license in accordance with physician orders, nursing standards of practice, NYS Department of Health regulations, and agency policies and procedures. This role serves as a clinical leadership position, responsible for the supervision, training, and ongoing development of nursing staff, oversight of patient care delivery, regulatory readiness, and participation in Department of Health surveys and site visits.
The Regional Nurse Manager may provide direct patient care as needed and may instruct Personal Care Aides (PCAs) under the agency's training program, including classroom and skills-based education, when operationally required.
Responsibilities:
Clinical & Regional Operations Oversight
  • Oversees all clinical operations for the assigned region, ensuring services are delivered in compliance
  • with NYSDOH regulations, agency policy, and accepted standards of nursing practice.
  • Provides clinical leadership and direction to nursing staff assigned to the region.
  • Ensures consistent implementation of Plans of Care and adherence to physician orders.
  • Serves as the primary clinical contact for the region for operational, compliance, and quality matters.
  • Participates in and supports NYSDOH surveys, audits, and site visits, including preparation, on-site
  • coordination, and follow-up as directed by the Administrator.

Field Supervision & Nursing Practice
  • Provides care and services according to scope and standards of Nursing Practice and adheres to the Code
  • of Ethics for Nurses.
  • Respects client and employee rights and privacy; ensures the security of protected information; practices
  • ethically and in compliance with the agency's compliance and privacy programs.
  • Follows agency infection control policies, including hand hygiene, bag technique, PPE use, aseptic
  • technique, exposure control, and equipment care.
  • Assesses and reassesses client needs and responses to care, treatment, and education.
  • Develops, implements, evaluates, and revises Plans of Care, including nursing care plans and aide care
  • plans, and ensures appropriate care delivery.
  • Completes all clinical documentation accurately, timely, and in accordance with agency policy and
  • regulatory requirements.
  • Coordinates patient care through communication with physicians, case managers, supervisors,
  • interdisciplinary team members, vendors, and community resources.
  • Supervises, mentors, and trains new and current nursing staff, including orientation, field supervision, competency validation, and ongoing performance feedback
  • Conducts or oversees nursing supervisory visits as required.
  • Provides patient and caregiver education based on assessed learning needs.
  • Interprets and reinforces agency philosophy, policies, and objectives to staff, patients, and families.
  • Performs other job-related duties as assigned by the Administrator.

Educator / Training Responsibilities
  • Delivers PCA classroom and practical training as needed based on agency operational needs, including core PCA skills and Alternative Competency Demonstration (ACD).
  • Utilizes appropriate teaching methods and technologies to support adult learning.
  • Demonstrates practical skills related to home care services and PCA scope of practice.
  • Evaluates trainee performance and provides constructive feedback.
  • Ensures all training activities comply with NYSDOH and other applicable regulatory requirements.
  • Maintains required training documentation, reports, and student records.

Education & Experience Requirements
  • NYS Licensed and currently registered as a Registered Professional Nurse.
  • Graduate of an approved school of nursing and holds a Diploma, Associate or Baccalaureate Degree.
  • Minimum of one (1) year of home care nursing experience.
  • RN education or staff training experience preferred.
  • Prior supervisory, management, or regional oversight experience strongly preferred.

Qualifications:
  • Strong clinical knowledge, nursing judgment, and leadership skills.
  • Demonstrates knowledge of NYSDOH regulations governing Licensed Home Care Services Agencies.
  • Demonstrated ability to supervise, train, and evaluate nursing and paraprofessional staff.
  • Excellent communication and interpersonal skills with the ability to provide clear direction and resolve
  • issues.
  • 2+ years of Home Care Nursing experience demonstrating comprehensive understanding of homecare complexities and challenges
  • Master's degree in nursing or related field preferred, showcasing advanced knowledge in healthcare management and leadership
  • Current license to practice as a Registered Professional Nurse in New York State, ensuring compliance with regulatory requirements and standards
  • Previous experience in a Leadership a role with a strong preference for previous DPS experience with strong ability to lead and manage a team effectively
  • Strong assessment, clinical and documentation skills, essential for ensuring high-quality patient care and regulatory compliance
  • Effective communication skills (written and verbal), facilitating effective collaboration with teams, patients and families
  • Strong attention to detail, ensuring accuracy in documentation and adherence to protocols
  • Highly organized, capable of managing multiple tasks and priorities efficiently
  • Motivated self-starter and creative problem-solver, comfortable navigating and resolving challenges in a fast-paced, dynamic environment
  • Proficient utilizing technology and systems to enhance operational efficiency and effectiveness in healthcare delivery

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $100,000 and $125,000 per year at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
