Healthcare Account Manager (Facility & Referral Networks)

Overview

This role is employed by Phoenix Med Health (MSO), a healthcare management organization supporting medical practices across operations, growth, and technology.

You will work directly on behalf of Active Vital Care (www.activevitalcare.com), a mobile, multi-specialty medical provider delivering in-home care to patients across assisted living facilities, group homes, and private residences.

We're hiring someone to own and grow our referral network across assisted living facilities, group homes, home health agencies, hospice, and other healthcare partners.

This is a growth-focused role responsible for driving patient volume through strong, consistent relationships.

What You'll Do
  • Own and grow relationships across facilities, home health agencies, hospice, and other referral sources
  • Represent Active Vital Care in the field as the primary point of contact
  • Drive consistent patient referrals and new patient registrations
  • Conduct regular in-person visits to partners
  • Identify and onboard new referral partners
  • Track outreach, referrals, and pipeline activity in CRM
  • Coordinate internally with scheduling, clinical, and operations teams
  • Resolve issues quickly and maintain strong partner satisfaction
  • Continuously increase referral volume per account

Requirements
  • 2+ years in healthcare account management, business development, or referral management
  • Experience working with facilities, home health, hospice, or similar referral environments
  • Comfortable being in the field regularly
  • Strong communication and relationship management skills
  • Highly organized with the ability to manage a high-volume partner pipeline
Nice to Have
  • Existing relationships with local facilities or agencies
  • Experience with CRM tools and EMRs
  • Understanding of the Medicare / senior care landscape
