Manages:ย ย ย ย ย ย  Caregivers & Clinical Support Staff

Pay Rate:ย  ย  ย  ย Up to $104,000 Annually with Bi-Weekly Pay & Benefits (Probationary Period and Policy Apply)

Duties include:

ยทย ย ย ย ย ย ย  Planning, organizing, directing and evaluating consumer care activities

ยทย ย ย ย ย ย ย  Drafting, modifying, and managing consumer care plans and ADL/IADLs

ยทย ย ย ย ย ย ย  Managing caregivers, consumer care oversight, home audits, and consumer QA visits

ยทย ย ย ย ย ย ย  Coordinating with case/care managers, private-pay families, and managing authorizations

ยทย ย ย ย ย ย ย  Monitoring, tracking, and documenting risk management activities

ยทย ย ย ย ย ย ย  Participating in community relations events and marketing campaigns for the agency

ยทย ย ย ย ย ย ย  Planning, facilitating, and managing all caregiver clinical training and continuing education requirements

ยทย ย ย ย ย ย ย  Protecting client and organization privacy and confidentiality

ยทย ย ย ย ย ย ย  Adheres to all PlanMerica, local, state, and federal policies and regulations, including HIPAA

ยทย ย ย ย ย ย ย  Others duties as assigned

Requirements:

A strong leader who is compassionate and extremely efficient to build a high-performing clinical team, effectively manage clinical operations, and achieve consistent consumer satisfaction.ย 

ยทย ย ย ย ย ย ย  Graduate of an accredited school of nursing.ย  Wound care certification is a plus.

ยทย ย ย ย ย ย ย  At least 2 years of experience in general nursing, one year of home health experience, preferred

ยทย ย ย ย ย ย ย  A minimum of one (1) year of demonstrated supervisory healthcare- related experience, required

ยทย ย ย ย ย ย ย  Candidate resume must reflect the healthcare-related supervisory experience in detail, including type of agency, job title, number and type of employees supervised, how long in supervisory role and specific details regarding the supervision. Resume must include the complete company name, address, city and state for each position.

ยทย ย ย ย ย ย ย  Active Nursing license in Indiana and/or hold a Compact license. GA and/or OH licenses are a plus.

ยทย ย ย ย ย ย ย  Knowledge of Medicare, Medicaid, and Commercial payer home health guidelines

ยทย ย ย ย ย ย ย  Willingness to work in home environments of patients who may have differing personal values or standards

ยทย ย ย ย ย ย ย  Ability to communicate effectively, both verbally and in writing

ยทย ย ย ย ย ย ย  Ability to lift up to 50 lbs in addition to turning, lifting, transferring, and transporting patient

ยทย ย ย ย ย ย ย  Current driverโ€™s license and active automobile liability insurance

ยทย ย ย ย ย ย ย  CPR, and First Aid certification

ยทย ย ย ย ย ย ย  Pass background check, medical and drug screening

ยทย ย ย ย ย ย ย  Computer and MS Office Suite of apps experience. (Cell phone with active service required for apps usage)
