Care Finders Total Care LLC, a Modivcare Home care company in NJ is looking for a Full Time RN Clinical supervisor in our Hackensack Branch. The RN field nursing supervisor is responsible to ensure quality and safe delivery of home health Care services such as supervising Certified Home Health Aides in a patient's home, evaluate the patient's plan of care and assess their physical and psychosocial needs.

Position Spotlight...

  • Schedule: FT - Monday through Friday 830a-5p
  • Office: Hackensack, NJ
  • Territory: Bergen County
  • Salary: 88K - 93K range

You will...

  • Conduct new patient admissions, comprehensive assessments, and supervisory visits in accordance with agency policies and state regulations.
  • Provide clinical oversight and support to caregivers through orientation, skills validations, and ongoing supervisory visits.
  • Perform routine and urgent patient visits to assess health status, evaluate care plans, and address clinical concerns.
  • Monitor patient progress and coordinate care with physicians, caregivers, and interdisciplinary team members.
  • Complete all clinical documentation accurately and within required timeframes.
  • Conduct follow-up activities including hospitalization reviews, patient wellness calls, and care plan updates.
  • Investigate incidents, patient complaints, and clinical concerns, implementing corrective actions as needed.
  • Support quality assurance, regulatory compliance, and survey readiness initiatives.
  • Assist with case management activities and ensure continuity of care for assigned patients.
  • Provide leadership support and field coverage as needed to meet patient care and operational demands.

  • Current RN NJ License
  • 1 year + professional experience as an RN in the United States preffered
  • Current CPR certification
  • Bi-Lingual (Spanish) Required.
  • Experience in Home Care Nursing Preferred
  • Ability to prioritize workload in an appropriate manner.
  • Excellent communication and analytical skills
  • Able to work in and create a team environment.
  • Effective time management and scheduling skills with a certain amount of flexibility
