Job Type:ย ย ย ย ย ย ย  Part-Time: 20 hours per week
Reports to:ย ย ย  ย Vice President of Care Management
About the Role:ย  As aย Care Manager, you will be a trusted guide and advocate for the clergy of the Catholic Diocese of Colorado Springs, helping them to navigate the complexities of healthcare and supporting their overall well-being. Rather than providing hands-on clinical care, youโ€™ll serve as a compassionate coordinatorโ€”ensuring that clergy members receive the right care, in the right place, at the right time.
This member advocate role blends purpose-driven service with professional expertise, offering the chance to build meaningful relationships while helping clergy members access the resources they need to flourish.ย  If youโ€™re passionate about supporting others, thrive in a mission-centered environment, and want to make a meaningful difference in the lives of those who serve the Church, this role offers a rare and fulfilling opportunity.
Key Responsibilities:
  • Promote preventive wellnessย by educating members on health initiatives and available resources.
  • Provide clear, accessible health informationย to help members understand and utilize their benefits.
  • Coordinate medical, dental, and specialist appointmentsย to ensure seamless care experiences.
  • Assist with remote monitoring toolsย such as glucometers, blood pressure cuffs, and alert systems.
  • Manage delivery of medical suppliesย and other essential health-related items.
  • Conduct home safety assessmentsย and arrange modifications or meal services when needed.
  • Connect members with community resourcesย including transportation, home care, and support services.
  • Guide members through benefit coordinationย and collaborate with plan design vendors.
  • Develop outreach campaignsย to increase awareness of available services and benefits.
  • Participate in client meetingsย to identify needs and ensure comprehensive support.
  • Provide post-hospitalization follow-upย including optional on-site visits.
  • Collaborate with pharmacists and social workersย to deliver holistic care support.
  • Maintain strict confidentialityย in accordance with PHI and HIPAA guidelines.
  • Engage respectfully and professionallyย with clergy members and colleagues.
  • Travel as neededย to member locations and events.
  • Perform additional dutiesย as assigned by the Vice President of Care Management.

Who Weโ€™re Looking For:
  • Minimum 3 years of experienceย in healthcare coordination or a related support role.
  • RN or LPN licenseย (active or inactive). Ideal for nurses seeking a meaningful non-bedside role.
  • Exceptional communication and listening skillsย with the ability to offer emotional support.
  • Strong organizational abilitiesย and attention to detail.
  • A humble, service-oriented demeanorย and genuine desire to support others.
  • Ability to work independently and collaborativelyย with healthcare professionals.
  • Proficiency in Microsoft Officeย (Word, Excel, PowerPoint).
  • Experience in clinical or medical office settingsย and a passion for helping individuals navigate the healthcare system.
