RN Care Navigator

Are you looking to make a POSITIVE IMPACT in your community? Do you believe nursing is more than tasks, that it is about connection, compassion, and making a difference?

Cheer Home Care is looking for an RN Care Navigator who loves spending time with older adults and provide high quality care in home. The RN Care Navigator collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, caregivers and nurses. They work closely with our office team and have excellent communication and problem-solving skills.

This position will be responsible for serving as the primary care manager, coordinating care to the clients among caregivers, RNs, therapists and outside representatives.

Option to work one weekend day and have one weekday off.
