About CRC:
For over 30 years, Community Rehabilitation Center (CRC) has proudly served Jacksonville by providing culturally competent mental health and wellness services focused on minority populations. Our comprehensive care addresses substance abuse, HIV care, and primary healthcare to improve quality of life through compassion and dedication.
About the Matrix Program:
Matrix House is a secure, in-jail treatment program for clients serving jail sentences with substance abuse or dependency issues. It offers four months of intensive treatment followed by 12 months of Aftercare, including counseling, education, support groups, and referrals to community resources.
Job Summary:
The Case Manager will provide aftercare services to program participants by assisting with educational/vocational enrollment, post-discharge planning, and job placement, ensuring ongoing support after release.
Key Responsibilities:
  • Provide case management and aftercare services with weekly contact for each client on the caseload.
  • Assist clients in enrolling in educational and vocational programs.
  • Conduct client visits in various settings, including home and work environments.
  • Support post-discharge planning addressing housing, personal care, medical, psychiatric, and employment needs.
  • Document all services timely and accurately in clinical records, complying with DCFS regulations and CRC policies.
  • Coordinate post-treatment services with other providers and family support as needed.
  • Develop and monitor individualized Client Continuing Plans.
  • Lead weekly Aftercare Support Group meetings.
  • Deliver training on addiction relapse triggers and prevention techniques.
  • Provide transportation for clients as necessary.
  • Establish and maintain community resource relationships.
  • Ensure smooth transition from Corrections Therapeutic Community to community reentry.
  • Maintain confidentiality and adhere to organizational core values.
  • Participate in weekly staff meetings, internal trainings, and Quality Improvement programs.
  • Other duties as assigned.

Qualifications:
  • Bachelor’s Degree in behavioral health or human services.
  • Minimum 2 years relevant experience.
  • Preferred: Certification in addictions or criminal justice professional fields.
  • Valid Florida driver’s license and clean driving record required.
  • Experience in social services preferred.
  • Eligible for Level 2 DCF Background Screening.
  • Strong communication skills, both verbal and written.
  • Proficient with computer systems and Electronic Medical Records (EMR).
  • Ability to manage confidential information and work effectively with diverse populations.
