Responsibilities
  • Prepare, monitor and coordinate the daily patient care activities of the unit to ensure all physician orders are implemented and all patient encounters are documented using the POMR format; schedules staff and patients to ensure full coverage of patient care at all times and to ensure effective patient services.
  • Establish lines of communications to verify community programs participation, streamline appointment scheduling for continued care during incarceration and follow up care upon discharge from correctional facility.
  • Develop discharge planning referral criteria for MAT intake from medical screeners
  • Consult with the Armor medical and behavioral health providers (including psychiatrist and licensed mental health professionals) on specific patients with substance abuse needs that require supplemental coordination in addition to individual treatment plans or behavioral health referral plans upon release from custody.
  • Participate in multidisciplinary planning in support specialized transitional programs dedicated to seriously or chronically ill patients (such as community based mental health teams and substance use programs.
  • Maintain statistical data of the MAT program to include:
    • Number of individuals inducted on MAT
    • Number of individuals continued on MAT
    • Number of day-of-release clients referred for treatment
    • Number of day-of-release clients admitted to community-based programs
    • Rates of MAT continuation at 30 days post release
Qualifications
  • Current state license as a Registered Nurse (RN)
  • Current CPR certificate
  • Proficient with electronic medical records
  • Ability to pass pre-employment Level 2 clearance to include background check and drug screen
