Case Manager - Russian Speaking | Brooklyn, NY • $25-$30/hour
About the Role A Brooklyn-based home care agency is looking for a compassionate, organized Care Coordinator to manage a caseload of 30 patients receiving home care services. This is a hands-on coordination role where you'll serve as the central point of contact for patients, families, and care teams - making sure every person on your caseload is getting the right care, at the right time, in the comfort of their own home. If you're someone who genuinely cares about the people you serve and knows how to navigate the home care space, this role is a meaningful opportunity to make a real difference.
Key Responsibilities
Patient Care Management
  • Manage an active caseload of 30 home care patients
  • Coordinate and monitor care plans to ensure patient needs are being met
  • Conduct assessments and follow up regularly with patients and families
Care Coordination
  • Serve as the liaison between patients, families, caregivers, and healthcare providers
  • Ensure seamless communication across all parties involved in each patient's care
  • Address and resolve issues or changes in patient condition promptly
Documentation & Compliance
  • Maintain accurate and up-to-date patient records within HHA systems
  • Ensure all documentation meets regulatory and agency compliance standards
  • Track authorizations, referrals, and service delivery
Qualifications
  • Russian language fluency required
  • Prior experience in home care case management or care coordination is a plus
  • Strong organizational skills with the ability to manage an active caseload
