This position is entirely remote, allowing you to serve our clients from the comfort of your home while maintaining a flexible schedule. Our work setting is rooted in hospice care, meaning you will be involved in multidisciplinary teams that focus on improving the quality of life for children and families during challenging times. Despite the remote setup, our team collaborates closely, using advanced tools and digital communication platforms to ensure that every team member feels connected and supported.

• Conduct comprehensive evaluations and assessments to determine the needs of children and families in hospice settings.
• Actively participate in the development and implementation of Individualized Family Service Plans (IFSPs), incorporating input from family members and other healthcare professionals.
• Monitor and document outcomes as a team member, ensuring that services provided align with best practices and regulatory standards.
• Provide Emotional Support and guidance to families, equipping them with strategies to enhance their child's development and navigate the complexities of their care.
• Utilize evidence-based practices in a family-centered approach, employing techniques inspired by Natural Learning Environment Practices.
• Adapt interventions based on each child's developmental status and individual needs, maintaining alignment with family goals and preferences.
• Ensure compliance with Federal and State regulations, safeguarding the health and safety of all individuals served.
• Develop strong relationships with children, families, and team members to foster an effective and supportive learning environment.
• Facilitate family education sessions, providing essential information and teaching essential skills related to child development.

Your primary focus will be on children facing significant challenges, including developmental delays and disabilities, as well as their families. Working within hospice settings means facing sensitive situations with compassion and understanding, providing essential support to families during life-changing moments. You will assess developmental milestones in early childhood and contribute to multidisciplinary approaches that serve young children, particularly in critical life stages.

• A Master’s Degree in Social Work (MSW) or a Doctorate of Social Work (DSW) from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in accordance with the Department of Public Health.
• One year of experience in early intervention or directly working with children and families in similar settings.
• Proficient in adapting interventions and learning styles to address each child's individual treatment plan effectively.
• Possession of a valid driver’s license to facilitate essential travel as needed.
• Outstanding communication skills, coupled with excellent organizational and time management abilities.
• Ability to collaborate respectfully with team members while maintaining a focus on client-centered care.

• Familiarity with trauma-informed care practices and Natural Learning Environment methodologies.
• Prior engagement in family-centered care and educational programs is beneficial.
