Job Summary

  • Promise Comprehensive Services, Inc. is accepting applications for a Relocation/Housing Stabilization Coordinator and Targeted Case Management positions.
We help clients find the right housing placements (Independent Housing, Independent Housing with visiting services, Assisted Living in one's own home, Assisted Living in shared single-family homes) for people living in nursing facilities, rehab institutions/ Transitional Care Centers who want to return to the community. Help seniors, people with disabilities, people experiencing homelessness or at risk of homelessness to find and keep housing. 
Tasks include: 
- Meeting with clients to do intake, 
- discussing housing choices 
- Help them complete housing applications. 
- Accompany them in touring potential housing,
- Accompanying and helping them during housing interviews, 
Continue to work with them even after they move to their new home by reminding them about the terms of their lease, paying rent on time, and generally avoiding things that might cause them to lose their housing. 
- Mediating between client and landlord when there is a problem. 
- Help them apply for programs they might be eligible for such as Supplemental Security Income, Disability benefits, Financial Assistance, and Food Support. 
- Assist clients get all the documents they need to apply for housing and benefits. 
- Accompany clients to all housing-related appointments. Help client move smoothly when housing has been found. Document all daily activities. Help clients access services that will help them live safely and more independently when they move back to the community and help them keep their housing.
*** We reimburse mileage at the end of the month. Work is both in the office and in the community.
  • QUALIFICATIONS
  • Bachelor's OR Associate Degree in Social Work, Psychology, Sociology, Human Services or Social Science, Education, Healthcare.
  • At least 2 year experience working in the field of Human Services, Social Services, or Healthcare. Or 2 years working with people who need help or with people with disabilities.
  • For applicants with a High School Diploma, you must have at least 3 years work experience in Human or Social Services
  • You must be organized, compassionate, and sensitive to diversity and people with disabilities.
  • You must also be a punctual, disciplined person who can make good decisions when faced with new challenges and can work effectively with minimum supervision.
  • You must know your way around the Twin-Cities and greater Metro area and be very resourceful and knowledgeable in accessing services and connecting clients to them.
  • You must be a compassionate person given the job involves providing services to people who have disabilities, mental health problems or other social problems.
  • Good computer skills. Must be very good in word processing and using the internet.
  • Good command of spoken and written English.
  • Clearance to work in the field of human services through a background check done by the MN Department of Human Services.
  • In addition to English, the ability to speak one of these languages will be a plus a plus: Spanish, Hmong, Somali, Sign Language.
  • Reliable vehicle with insurance and a valid driver's license.
