Location: Delaware (New Castle County) - Field-Based / Remote Hybrid
Duration: 6 Months (Potential Extension/Conversion)
Schedule: Monday - Friday, 8:00 AM - 5:00 PM EST
Pay Range: $35 - $41 hourly on W2
Job Summary
We are seeking an experienced LTSS Service Care Manager (RN) to support long-term services and supports (LTSS) members through comprehensive care coordination and case management. This role focuses on evaluating member needs, facilitating care plans, and ensuring access to appropriate healthcare and community resources to drive quality and cost-effective outcomes.
Key Responsibilities
  • Assess member needs and develop individualized care/service plans
  • Conduct level of care redeterminations and support transition of care activities
  • Coordinate with healthcare providers, specialists, and community resources
  • Facilitate communication between members, families, caregivers, and care teams
  • Perform home visits to assess member conditions and service needs
  • Monitor care plans and adjust based on member progress and outcomes
  • Provide education on healthcare services, benefits, and available resources
  • Assist members with needs such as housing, employment, and independent living support
  • Maintain accurate documentation in compliance with regulatory requirements
  • Collaborate with internal teams to improve care quality and service delivery
Required Qualifications
  • Bachelor's degree or equivalent experience
  • 2-4 years of relevant experience in care management or case management
  • Active Delaware or Compact RN License
  • Valid driver's license and ability to travel for home visits
  • Experience with level of care assessments, transition of care, and case management
  • Strong computer proficiency
Preferred Qualifications
  • Experience working as a Service Care Manager
  • Experience evaluating elderly or disabled populations
  • Knowledge of medication reconciliation processes
