A-Line Staffing is now hiring LTSS Service Care Managers (RN/LCSW) in Greensboro, NC. The LTSS Service Care Manager would be working for a Fortune 500 company and has career growth potential. This would be full time / 40+ hours per week.


Hiring in Guilford, Pitt, and Cumberland county!


LTSS Service Care Manager (RN/LCSW) Compensation

  • The pay for this position is $38-$40 per hour based on experience
  • Benefits are available to full-time employees after 90 days of employment
  • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

LTSS Service Care Manager (RN/LCSW) Highlights

  • This position is a 6-month contract assignment with potential to extend and convert to permanent based upon attendance, performance, and business needs
  • Remote position for candidates living in one of the following North Carolina counties:
    • Guilford County
    • Pitt County
    • Cumberland County
  • Schedule: Monday-Friday, 8:00am-5:00pm EST
  • Training is Monday-Friday, 8:00am-5:00pm EST for approximately 3.5 weeks
  • Remote with 1-2 administrative work-from-home days per week
  • Field-based member visits 3-4 days per week (up to 80% travel)
  • Mileage and lodging reimbursement provided when applicable
  • Valid driver's license required

LTSS Service Care Manager (RN/LCSW) Responsibilities

  • Manage a caseload of Long-Term Services and Supports (LTSS) members
  • Complete comprehensive member assessments and develop individualized care plans
  • Coordinate healthcare services with providers, caregivers, and community resources
  • Conduct in-person member visits and telephonic follow-up
  • Monitor care plans to ensure members receive appropriate services without gaps
  • Authorize referrals and coordinate long-term care services
  • Collaborate with hospitals, discharge planners, physicians, and interdisciplinary care teams
  • Assist members with complaints, appeals, and access to available benefits
  • Maintain accurate documentation in the electronic medical record system
  • Participate in virtual team meetings and collaborate with internal care management staff

LTSS Service Care Manager (RN/LCSW) Requirements

  • Active Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW/LCSW-A) license
  • Bachelor's degree required
  • 2+ years of physical health case management experience
  • Experience with care planning and coordinating member services
  • Hospital case management, home health, utilization management, discharge planning, or community health experience
  • Valid driver's license and willingness to travel for member visits
  • Comfortable using Microsoft Office and electronic medical records
  • High School Diploma or GED
  • Attendance is mandatory for the first 90 days

LTSS Service Care Manager (RN/LCSW) Preferred Qualifications

  • 4-6+ years of physical health care management experience
  • Home health or community-based case management experience
  • Hospital bedside case management background
  • Experience working with LTSS populations
  • Strong communication, documentation, and organizational skills
  • Comfortable working independently in a remote environment while managing a field-based schedule
