The Associate Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service and adhere to Lean processes. Supports the teams in meeting financial, clinical and service goals.

Primary Responsibilities:

  • Call patients to remind them of scheduled appointments
  • Support the Clinical team in chart preparation and request medical information
  • Explain the process of the wellness visit to patients including, but not limited to what is involved, who the provider is, and the location of the specialist office. Ability to directly schedule patients into all listed clinics to ensure a seamless transition for patients
  • Responsible for obtaining 'expert' level knowledge of the inter-workings of the Medicare Advantage plans
  • Responds effectively to incoming patient calls and communicates pertinent information to designee to provide for continuity and optimal care
  • Attends departmental meetings and provides constructive recommendations for areas of improvement
  • Documents pertinent patient information in the electronic medical record as it relates to job function
  • Coordinate with various providers, team members and outreach team regarding any scheduling guidelines
  • Meets established productivity, schedule adherence, and quality standards
  • Uses, protects, and discloses Optum patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
  • Performs additional duties as assigned

Required Qualifications:

  • High School Diploma/GED
  • 1+ year of medical front office experience
  • Intermediate level of computer proficiency (multi-task through multiple applications including Microsoft Outlook, Excel, and Word)

Preferred Qualifications:

  • 1+ year of related healthcare experience, including scheduling experience
  • Knowledge of medical terminology
  • EMR experience
  • Office Administrative / Receptionist / Clerical experience in a medical environment

  • Ability to manage time effectively and work independently

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $16.00 to $29.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
