Direct Support Professional

Mentor, OH

Helpful Hearted Providers Inc. is currently seeking part- and full-time Direct Support Professionals (DSPs) to join their team in Mentor, Ohio! In this role, you will be responsible for providing person-centered supports to individuals with developmental disabilities in accordance with their Individual Service Plan (ISP). DSPs assist individuals in maintaining their independence, ensuring their health and safety, and supporting them in participating fully in their home and community.

  • Pay: $16 - $19/hour, offer dependent on experience

Shifts:

  • 2nd shift: 4PM – 12AM
  • Part- and full-time available

The right candidate will have:

  • High school diploma or GED required
  • Must be 18 years of age or older
  • Valid driver’s license and reliable transportation preferred
  • Ability to pass BCI background check
  • Ability to obtain required certifications including CPR / First Aid, Medication Administration Certification, DODD training requirements
  • Compassion and patience when working with individuals with disabilities
  • Strong communication, interpersonal, problem-solving, and observation skills
  • Ability to follow service plans and care instructions while working independently and as part of a team

Physical Requirements:

  • Ability to stand, walk, bend, and lift as needed
  • Ability to assist with transfers and mobility support
  • Ability to respond quickly in emergency situations

Work Environment

DSPs may work in residential homes, community settings, or individual residences and may provide services during day, evening, overnight, weekend, or holiday shifts depending on the needs of the individuals served.

Our Direct Support Professionals will:

  • Provide Homemaker Personal Care (HPC) and other waiver services as outlined in each individual’s ISP.
  • Assist individuals with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and meal preparation.
  • Support individuals with mobility, transfers, and safe transportation when required.
  • Ensure health and safety monitoring, including observing and reporting changes in behavior, health, or environment.
  • Administer medications when medication administration certification is obtained and in accordance with agency policy.
  • Assist individuals in community activities, appointments, recreational activities, and social engagement.
  • Follow all DODD rules, Medicaid waiver requirements, and agency policies.
  • Maintain accurate and timely documentation, including service notes, incident reports, and daily logs.
  • Implement behavior support strategies as outlined in the Behavior Support Plan (BSP) if applicable.
  • Communicate effectively with supervisors, families, guardians, and team members.
  • Maintain confidentiality and respect individuals’ rights and dignity at all times.
