Job description

Library Outreach Case Manager

Summary

This position is responsible for providing outreach to assigned EBR Library

Position Type

Non-Exempt/Full Time

Essential Functions

· Provide linkage & referrals to community services in EBR

· Maintain sufficient knowledge of community resources

· Provides liaison services between assigned EBR library staff & patrons

· Document outreach contacts on appropriate forms

· Refer individuals to eligible services in areas of housing, job training, mental health, substance abuse, after school programs, elderly services, health & education, etc.

· Follow all guidelines & procedures related to payroll including but not limited to submitting contact logs by payroll deadline.

· Report any significant concerns to Supervisor

· Maintain training requirements

· Maintain assigned program records.

· Projects and duties as assigned

Required Education and Experience

Bachelor's degree in a Social Work and one year of post graduate work experience is required. Candidates must also have excellent verbal & writing skills and knowledge of various community resources. Local travel is required, so candidates must have a valid driver's license, auto insurance, reliable transportation & clean MVR record are required. Qualified candidates must be self-motivated as well as the ability to prioritize and multi-task.

While performing the duties of this job, the employee is frequently required to stand; walk; sit, use hands, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this job, the employee can sometimes be exposed to weather conditions. The noise level in the work environment is usually moderate.
